Better Choices, Better Health: Managing Chronic Conditions

Asbury is proud to be one of only 13 organizations in the state of Pennsylvania licensed to offer the Stanford Chronic Disease Self-Management Program.

This program consists of a free six-week workshop offering tips and ideas to help make life easier for those facing one or several ongoing health issues. Each workshop is approximately 2 ½ hours.

Our next workshop begins Monday, April 16 from 1:00 PM until 3:30 PM. Each participant will receive a textbook titled “Living a Healthy Life With Chronic Conditions.” Those who complete at least four of the six sessions will receive a relaxation tape.

This program is a partnership between the Allegheny County Area Agency on Aging, the United Way of Allegheny County and Vintage, Inc.

For nearly 20 years, Stanford has developed a variety of self-management programs for those suffering from chronic conditions. Their programs (including Better Choices, Better Health) are designed to help participants regain self-confidence and control of their lives.

Stanford’s programs are evaluated for their effectiveness and safety through a trial period of 2-4 years before they are available to the public.
